Ingestion of alcohol-containing medications will increase one's blood alcohol concentration. But, you'd have to ingest a lot of it -- on the order of bottles of cough syrup -- to get a measureable change. Employer policies do not try to determine the source of alcohol and instead just set a threshold level at or above which there are consequences.
