Most collection sites use a release / consent form for each drug test specimen collection, and use the same form whether the collection is DOT or non-DOT. DOT does not prohibit use of such forms, but also does not allow employers to insist that such forms be completed, nor can the employer or clinic use the form to ask the donor for a "hold harmless" or other waiver of liability. While we each may criticize certain aspects of certain forms, I think collection sites believe release / consent forms serve to educate the donors with regard to how the test is performed and what happens to the result. Furthermore, use of such forms is so ingrained into medical practice, collection sites (which are usually health care providers, too) are likely to also use these forms for their drug test specimen collections.
