I also have recently been getting a whole lot of urine results like this.  These results amount to urine specimens that are invalid. We report them out,"Invalid, immediate observed collection authorized without advance notice to the donor." I try to get a confession from the donor.  Sometimes I can and sometimes I can't. When I can't, it gets reported as invalid. When I can, it gets reported as substituted. Hope this helps.
