To recap: You're the MRO. You recd a cold, negative specimen and the collector says the donor refused to submit a second specimen. It's particularly important to assess the strength of the collector's documentation in this regard. If the collector has not written anything down about this, then immediately suggest to the collector that he or she write a detailed narrative about what happened. The donor sometimes remembers these things differently, and it's therefore important that the collector document in writing specifically what happened, hopefully at the time that it happened (or soon thereafter). Once documented, review it and, if it's sufficient, report it as a refusal to test. If it's not sufficient, inform the employer that the donor needs to immediately return for a second collection under direct observation. And, oh yes, report the cold negative result as "negative."
