Use of DOT CCF for non-DOT test

DOT form used for non-DOT test.  The first lab said the test was invalid, possibly adulterated.  An aliquot was then sent to a second lab, which found iodine.

Can I correct the error of using a DOT form on a non-DOT test?  Please cross-reference your reply to the DOT Procedures, if possible.
