Under the DOT Procedures, 49 CFR 40.203(d)(3), use of a non-DOT form for a DOT test is a correctable flaw. The DOT Procedures do not directly address the use of DOT forms for non-DOT tests, since DOT does not regulate non-DOT programs. Non-DOT programs shouldn't use Federal CCFs because those CCFs reference federal rules that may not apply. There is no regulation that requires a corrective statement for miss-use of the DOT form, but it may be worth considering for purposes of setting the record straight and re-educating the collector about the mistake.
