Hydrocodone metabolism to codeine has not been reported.
By contrast, there is one research study that reports the detection of low concentrations of hydrocodone (~100 ng/mL) in urine of codeine users containing high concentrations of codeine (more than 5000 ng/mL) [Oyler et al. JAT. 2000;24:530-5]. That study's findings suggests the possibility that codeine use may result in the presence of hydrocodone in urine, i.e., codeine --> hydrocodone.
