Texas also requires a report and has for about a year and a half if I remember correctly. This is only for CDL holders from Texas who fail a Federal Drug test. The program is similar to Oregon and Washington except that the report is filed via the Employer in Texas. The MRO completes the top page of the form, the Employer the bottom part and then the Employer sends it to the State.
