If this driver is on hemodialysis you would certainly want to make certain the examiner is aware of the medical condition and the potential safety concern. A recent "frequently asked question" (FAQ) from the Federal Motor Carrier Safety Administration (FMCSA) addresses dialysis:
At this time, there is no guidance from FMCSA. At the least, the examiner should require a letter from the treating doctor (nephrologist) outlining the condition, medications, and recommendation regarding certification. The examiner may or may not accept this recommendation. The driver must meet all criteria for certification. Restrictions other than those on the pre-printed form are disqualifying.
An examiner cannot restrict duties (i.e., can only drive on days when not being dialyzed). And, another FAQ indicates that when an examiner signs the certificate it is NOT employer specific.
