By "reliable," I'm assuming you mean, How specific is a GC/MS confirmed oral fluid test result for a specific drug/metabolite? The answer is, essentially 100% specific.
DOT is not considering use of oral fluid test technology. HHS was considering release of guidelines for use of oral fluid tests in federal agency testing programs, but has put those guidelines on moth balls because of concerns about those guidelines and perhaps a shift in government priorities during the past few years. If/when HHS issues final guidelines, DOT will consider whether to incorporate those guidelines into its programs. My prediction: It won't happen within the next five years.
