I was hoping Natalie would chime in - on that letter requested from the nephrologist by the DOT examiner (not the MRO) I would suggest your language not simply be "recommendations regarding certification" but rather "any recommendations for restrictions on driving activities or duties" for a a more insightful reply. The time off for dialysis in a successfully treated compliant patient is not an issue to me (similar to any medical care visit and able to be scheduled) but no driving for a period after dialysis certainly raises a safety issue which isn't self limiting.
