your complaints are valid but the remedy is elusive.  When does he present the note? to whom? who verifies? how long is it valid for exemption?  (when was the transplant?)  is all of this done by the collection site personnel?

Quality is best assured by nonvariance (i.e., always done the same way), which the current method utilizes for all, making it the simplest way to accomplish the task with the complexity of a rare situation.  There are a lot of websites arguing that anyone having to spend time for a drug test is an unfair infringement of personal freedoms.
