Refusal to test in an owner-operator (DOT)

We had a situation where an owner-operator left the collection site facility after a shy bladder protocol had begun for a random (not pre-employment) federal drug screen. My questions are:

(1) My interpretation is that I can mark this as a refusal to test based on collection-site events. The question was raised as to whether I should interview the donor before marking it as a refusal to test? Is this interview needed?

(2) Who else should I report the refusal to test to in addition to the owner-operator?   If the owner refuses to disclose the TPA, should I report it to the FMCSA?
