Positive for multiple opiates

I have a reasonable suspicion HPP3 panel positive for oxycodone, oxymorphone, hydrocodone, hydromorphone and fentanyl.  The test was ordered "reasonable suspicion" because of Dilaudid that went missing.

It is my understanding that hydrocodone (Vicodin) metabolizes to hydromorphone.  In this test, the levels of hydrocodone and hydromorphone are 9600 ng/mL and 4200 ng/mL, respectively.  It is also my understanding that oxycodone (Percocet) metabolizes to oxymorphone.  The oxycodone and oxymorphone levels are 2000 ng/mL and 700 ng/mL, respectively.

The fentanyl level is 375,000, and this donor tells me that he helps a relative put on the relative's fentanyl patch.  I can accept the first two results, as long as I receive proper supporting documentation.  But, what are your thoughts about the fentanyl?
