If the donor has a legitimate Rx for hydrocodone, this would explain the hydrocodone/hydromorphone. Similarly, if the donor has a legitimate Rx for oxycodone, this would explain the oxycodone/oxymorphone.
It sounds like this donor does not have a Rx for fentanyl. The history suggests that he is using someone else's medication, and not just putting on their patch. If the donor does not have a legitimate Rx for fentanyl, I would report this out as positive.
