Absent name, social security number, reason for test

At the top of the form, the donor name, social security number, and reason for the test are all left off of a drug screen collected in the ER. The donor did sign the bottom of the form appropriately. Do I just have the collector sign an affidavit, or is this too much missing information? The specimen numbers match on the lab report and the chain of custody. This is a non-regulated, post-accident test. Thank you.
