I would attempt to get the affidavit.  Do the medical review as usual but attempt to confirm other info -- e.g., birth date and specimen ID on donor copy (oh he has it!) to further tie the specimen to the donor -- and report it.  Perhaps with notation about missing ID number and reason for test - you actually do have the donor's name.  If later additional defense needed, I expect/hope you have the donor's initials on the specimen seals as a further evidence of for whom this CCF tolls.
