It is reported that high levels of morphine can cause low levels of hydromorphone to show up on urine drug tests. We received a result with morphine > 20,000 and hydromorphone 488 ng/m. The donor has a prescription for MS Contin but denied use of hydrocodone (which is metabolized to hydromorpone) and denied use of Dilaudid.  Would you downgrade the hydromorphone-positive result based on the high morphine level?  If so, at what level would the hydromorphone be too high to overturn based on the morphine?
