For a lab invalid: If the "why" is truly known and is a legitimate explanation, the test is simply canceled (a no-test result) - observed collection not required [though routine recollect if a negative result is required, e.g. preemployment or RTD].  If the "why" is truly known and is not a legitimate explanation, it is a Refusal/Adulteration.  If the "why" is unknown or uncertain, the result is Test Canceled, Invalid due to specimen characteristics, and requires (if DOT -- otherwise recommended)immediate observed recollect.  I believe that is the case here.
