Many MRO's routinely get isomer testing.  You said you infrequently need them, suggesting you base it on plausible interview.  (I fall into that camp, too.)

My suggestions / possible solutions:

1) bundle isomer testing into the lab quote so it's not a separate charge (when bundled the cost is negligible)

2)When you have reason to believe isomers are needed and not obtained on a non-regulated report it with a comment e.g. "further testing required to discern legitimate medical from illicit use"

3) On regulated there are times I would insist on isomer testing and the employer is responsible for the cost of testing (unless your facility agrees or contracted to absorb it - often on DOT MRO the lab bill goes elsewhere anyway!).
