Donor's name on the lab copy

If an MRO assistant has written the name on the lab copy of the results before the MRO signs the MRO form, is this a violation of DOT regulations?   It is my understanding of the DOT rules that this should not take place, that the lab copy should never have the donor's name on it, either printed or written.  Please advise and thank you very much!
