Dr. Doug. The bottom line when it comes to verifying drug test results depends on what I feel I can defend, with a high degree of success, in front of a judge. In a case like this, there is a good likelihood that this might end up where you would have to defend your position at a legal proceeding or hearing. In light of the fact that this donor was essentially caught in the bathroom where Dilaudid tablets were found wrapped in a paper towel in the wastebasket, and in light of the levels, I would feel comfortable verifying this as a positive test for Hydromorphone. Getting back to levels. I have found that as you get closer and closer to the lower levels, that the hydromorphone level can surpass the hydrocodone levels. This rarely happens with higher levels. Since the hydromorphone level is over 6 times the hydrocodone level, and in light of the surrounding facts, I would feel comfortable reporting, in this case, that the Hydromorphone is not solely from the Hydrocodone.
