You can use 99080. If your billing system attaches a price to a CPT code, and if there's already some other price in there for 99080, then hopefully the system will allow you to use a made-up code like 99080F (for "fit test:).

The state licensing board may have a right to request and receive the test result directly from you. The agency doesn't, unless the donor has signed a release form authorizing you to give the result to the agency.

Yes for EtG and EtS, especially at the lower concentrations, e.g., at EtG of <500 ng/mL.

I am not aware of such exposure causing positive results for ethanol.

The abstract and references at https://www.nature.com/articles/s41433-019-0486-z seem relevant. I suspect this issue has been raised in some of those references, but doubt you'll find a black and white answer to the question.
