Heavy equipment operator w/a stroke or TIA

Fitness for duty evaluation of a 53 yo non-DOT worker who drives excavator, skid loader, tire loader, haul truck at work. He was diagnosed with CVA/TIA episode in Nov 2017, and history of two previous episodes, patent foramen ovale on Xarelto and Plavix. Got clearance from his neurologist (who understands he drives heavy machinery) to go back to work without restrictions.

What would you do?

I usually treat these by DOT guidelines. Even if he currently has no symptoms or signs, should we wait one year?

Yes. The DOT publication(s) that recommend a wait period do so based on risk of recurrent TIA or stroke, not on the patient's symptoms or signs.

For items such as this for fork lift operators, we would 1) get an opinion from the treating physician for likelihood of recurrence (some physician's do not realize actually what their patients do for a living) 2) insure adequate time had passed (we usually restrict them for about 6 months to see if they have a recurrence and 3) have the demonstrate competence by performance.
