Respiratory mask fit test

we have a construction contractor wanting to send his employees to us for their respirator evaulations ( mask fit test).

What CPT code should be used?

There is no CPT code for respirator mask fit tests.

Where the electronic medical record and/or billing service requires a billing code for every encounter, you'll need to create faux-codes (with associated real prices) for services like this. For example, assign to the service 99080 "Special reports such as insurance forms, more than the information conveyed in the usual medical communication or standard reporting form," associated with your usual fee for mask fit tests.

How much, if I may ask, is the usual fee for this service. I have been doing this before but the cost is bundled for the total service. My new employer is asking me to price the actual review of the questionnaire and I have no idea. Can anybody help? Can somebody break down the cost?

You can use 99080. If your billing system attaches a price to a CPT code, and if there's already some other price in there for 99080, then hopefully the system will allow you to use a made-up code like 99080F (for "fit test:).
